ImPACT Passport is a free concussion care app where you can store your unique 12 digit code assigned after completing an ImPACT®, ImPACT Pediatric®, ImPACT Quick Test®, or Cognitive Impairment Screener™ (CIS). When it’s time to seek care, share your ImPACT Passport ID with your healthcare provider. Test results can only be viewed in Customer Center the by trained clinicians to keep your information secure.

Test takers or guardians can store ImPACT Passport IDs in the app and share it with healthcare providers to grant access to clinical reports for ImPACT, ImPACT Pediatric, ImPACT Quick Test, and Cognitive Impairment Screener (CIS). Easily upload your ID by scanning the QR code found on your test completion confirmation.

ImPACT PASSPORT FAQs
SHOULD I USE MY FULL NAME OR NICKNAME?
The information you enter in the ImPACT Passport app must match the name as it was entered during the test, including shortened names. Also, be sure there are no extra spaces before or after your first and last name.
WHY CAN’T I ADD ImPACT
PASSPORT ID?
Confirm you are inputting correct digits for the ImPACT Passport ID. Common mistakes include putting in the letter ‘o’ for the number ‘0’, and capital ‘i’ in place of 1.
CAN I SEE MY
TEST RESULTS?
The ImPACT Passport app only stores your name and the ImPACT Passport ID. It does not retrieve test results. Use the app to find a concussion care specialist who can interpret your results.
